29 CFR 1926.1404(q)(5)(i): The outrigger and stabilizer blocking did not meet the requirements in paragraphs (h)(2) and (h)(3) of this section.    On or about December 19, 2013, during setup assembly of the Manitex truck mounted crane, the employer did not ensure the blocking material was sufficient to sustain the loads and maintain stability exposing employees to struck by and crushed by hazards:    a) under the rear drivers side outrigger, the employer used Oriented Strand Board (OSB) sheathing as a base under the blocking;    b) on the right front outrigger, the stabilizer pad and blocking was not placed on level ground to prevent movement or collapsing.
