OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to electrical hazards.    On or about December 19, 2013, and times prior thereto, at the work site located at E & H Drilling, Rig #1, HWY 114, Jacksboro, Texas: The employer did not ensure that all trailers including but not limited to the Dog House and Shale Shaker trailer were grounded exposing employees to electrical hazards.

29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded.   On or about December 19, 2013, and times prior thereto, at the work site located at E & H Drilling, Rig #1, HWY 114, Jacksboro, Texas:  a 2 feet x 2 feet floor panel section at the southeast corner was missing from the platform exposing employees to a fall hazard of approximately 10 feet.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards: Note:  Toe boards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.   On or about December 19, 2013, and times prior thereto, at the work site located at E & H Drilling, Rig #1, HWY 114, Jacksboro, Texas:  employees involved in oil and gas drilling operations worked on the platform with an opening at the southeast corner that was not guarded with a standard guardrail or chain exposing employees to a fall of approximately 10 feet to the ground below.
