29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift.   On or about December 23, 2013, on the south side of the commercial building, an employee installing an outdoor sign on a commercial building worked from a vehicle-mounted rotating platform and was exposed to a fall hazard of approximately 12 feet.
