29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.      On or about December 17, 2013, on the west side of the construction site, an employee engaged in framing activities on a residential home worked from a pole scaffold platform without fall protection and was exposed to a fall hazard of 14 feet to the ground below.

29 CFR 1926.452(a)(4): Diagonal bracing in both directions were not installed across the entire outside face of all double- and single-pole scaffolds.      On or about December 17, 2013, on the west side of the construction site, an employee engaged in framing activities on a residential home worked from a pole scaffold platform supported by a single pole frame that did not have cross braces exposing the employee to a fall hazard approximately 14 feet to the ground below.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structural, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.       On or about December 17, 2013, on the west side of the construction site, an employee engaged in framing activities was not provided a ladder for access to the upper level exposing the employee to fall hazards of approximately 14 feet to the ground below.
