At the worksite:  On or about December 11, 2013, the employer allowed employees to conduct work activities while exposed to head injury hazards.  Employees were exposed to striking the head hazards.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm from struck-by falling objects:    At the worksite:  On or about December 11, 2013, the employer allowed the use of a rod hook assembly without the safety latch in safe working condition.  The employee was exposed to the hazard of struck-by falling objects.
