29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about December 27, 2013, the magnetic interlock on the FlowMaster 1 was defeated on the clear cover guarding the insert entry finger assembly, allowing the cover to remain open while the machine was operated. b) On or about December 27, 2013, the magnetic interlock on the FlowMaster 2 was defeated on the clear cover guarding the insert entry finger assembly, allowing the cover to remain open while the machine was operated. c) On or about December 27, 2013, the magnetic interlocks on the inTelmail inserter were defeated allowing the covers to remain open while the machine was operated. Abatement Certification and Abatement Documentation Required.
