Gaeta, LLC located approximately at 2215 Leavenworth Street, Omaha, NE:     The employer failed to keep an employee clear of a suspended load in that on or about December 27, 2014 an employee of TM Gaeta, LLC was struck in the back of the head with the bucket of a Ford FZ111V Front- End Loader (model# FZ111V tractor# C446257). The employer was using chain sling attached to an anchor point on the bucket of the loader to lift/move 36" wide, 60" long and 25" tall concrete retaining wall blocks. The employee was attempting to grab/attach the sling to the anchor point on a retaining wall block from underneath the elevated bucket of the loader.
