OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that were causing or likely to cause death or serious harm to employees in that were exposed to the hazard of crushing injuries due to inadequate automotive lift annual inspections:    (a)Best West Tire & Service Inc., 600 E 8th Ave, Yuma, CO 80759:  On and before December 30th, 2013, Best West Tire & Service Inc., did not ensure that the center jacks on blue post-lift were operating properly.  Employees use the Worth 4 post Lift, Serial No. 023272010900, Model 12000E to perform maintenance on larger vehicles when the other lifts are in use. The lift is located at the east end of the mechanics bay. This condition exposed employees to crush-by hazards.    (b)Best West Tire & Service Inc., 600 E 8th Ave, Yuma, CO 80759:  On and before December 30th, 2013, Best West Tire & Service Inc., did not ensure that the safety locks on the lifts were operating properly.  The safety lock on the Benwil 2-post lift, Serial No. 5559, Model LP-7, Capacity 7000lb does not lock properly. The lift is used with smaller vehicles and located on the west end of the mechanics bay. This condition exposed employees to crush-by hazards.    Among other methods, feasible means of abatement methods to correct this hazard is to follow section 5.1., Periodic Qualified Inspection Procedures listed in the American National Standards Institute/Automotive Lift Institute ALOIM-2000, "Safety Requirements for Operation, Inspection, and Maintenance."    Abatement Note: Paragraph 6.1.2 in the American National Standard Institute publication number ALOIM-2000, lists the minimum requirements a preventative maintenance technician must possess prior to performing preventative maintenance on automotive lifts.

This condition exposed the employees to the hazard of electrical shock and burns.    (c) Best West Tire & Service Inc., 600 E 8th Ave, Yuma, CO 80759:  On and before December 30th, 2013, Best West Tire & Service Inc., a 125V 15A duplex outlet contact stab opening on the east shop wall next to the fire extinguisher was damaged with the inner electrical components exposed. This condition exposed the employees to the hazard of electrical shock and burns.    (d) Best West Tire & Service Inc., 600 E 8th Ave, Yuma, CO 80759:  On and before December 30, 2013, Best West Tire & Service Inc., a 125V 15A duplex outlet contact stab opening on the west wall of the oil and tire storage room was damaged with the inner electrical components exposed.  The face-plate was also missing with a hole allowing contact with the inner conductors. This condition exposed the employees to the hazard of electrical shock and burns.
