CFR 1926.451(c)(2):  Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  On or about 12/23/13 employees were exposed to a fall hazard from approximately 14 feet to the unpaved ground level below while doing masonry work from a scaffold that had the baseplates resting directly on the ground and not on mud sills.

29 CFR 1926.454(a): The employer did not have each employee who performed work on a scaffold be trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used, and to understand the procedures to control or minimize those hazards:  On or about 12/23/13 employees were exposed to a fall hazard from approximately 14 feet to the unpaved ground level below while doing masonry work from a scaffold that had the base plates resting directly on the ground and not on mud sills.
