29 CFR 1926.502(b)(2):  Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structural members shall be installed between the top edge of the guardrail system and the walking/working surface when there is no wall or parapet wall at least 21 inches (53 cm) high:    (a)  Shambaugh & Son, L.P. at 10241 E 51st Ave, Denver, CO 80239:  On and before December 16, 2013 the exposing employer did not ensure midrails and intermediate vertical members were installed in one area located inside the building on the second level.  Employees performed electrical-related work at that location.  This condition exposed employees to a fall of approximately 18 feet.

29 CFR 1926.502(b)(3):  Guardrail systems shall be capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge.     (a)  Shambaugh & Son, L.P. at 10241 E 51st Ave, Denver, CO 80239:  On and before December 16, 2013 the exposing employer did not ensure that guardrails were capable of withstanding, without failure, a force of at least 200 pounds, in that sections of toprail was missing at various locations inside the building on the second level.  Employees performed electrical-related work at those locations.  This condition exposed employees to a fall of approximately 18 feet.

29 CFR 1926.502(b)(5):  Midrails, screens, mesh, intermediate vertical members, solid panels, and equivalent structural members shall be capable of withstanding, without failure, a force of at least 150 pounds (666 N) applied in any downward or outward direction at any point along the midrail or other member:     (a)  Shambaugh & Son, L.P. at 10241 E 51st Ave, Denver, CO 80239:  On and before December 16, 2013 the exposing employer did not ensure plastic mesh used as a midrail was capable of withstanding, without failure, a force of at least 150 pounds, in that the mesh was not secured at its base.  This condition exposed employees to a fall of approximately 18 feet.
