29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about January 6, 2014 and at times prior thereto, the employer had an energy control program with procedures dated 5/10/2007 for the locking and tagging equipment that had no record or evidence of having been reviewed annually.  Employees were exposed to electrical, struck by, and caught in hazards.
