29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.       The employer does not ensure that machine guarding is provided to protect the operator from hazards such as those created by rotating parts.  This violation was observed on or about January 10, 2014, in the drive shaft rebuild shop where the lathe operator was exposed to struck by hazards when operating the Lehman lathe without a guard over the rotating chuck.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that guards are installed the lathe to protect employees from the rotating parts.

This violation was observed on or about January 10, 2014, in the drive shaft rebuild shop where employees were exposed to struck-by hazards when operating a Westward Bench Grinder that did not have a work rest.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that work rests are installed on all grinding equipment used for offhand grinding work.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:   The employer does not ensure that the distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top does not exceed one fourth inch. This violation was observed on or about January 10, 2014, in the drive shaft rebuild shop where employees were exposed to a struck-by hazard when operating a Westward Abrasive Wheel Bench Grinder that did not have a tongue guard installed and the end of the safety guard peripheral member at the top exceeded one fourth inch.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top does not exceed one fourth inch.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:   The employer does not ensure that pull boxes, junction boxes, and fittings were provided with covers approved for the purpose.  This violation was observed on or about January 10, 2014, in the drive shaft repair shop where employees were exposed to electrical hazards while working near a junction box that had no cover and that had energized conductors.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all junction boxes have approved covers.
