29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:     On or about January 9, 2014, an employee at Alumi Tank, Inc., was required to wear a respirator during polishing and buffing operations.  The employer did not establish and implement a written respiratory protection program.     DOCUMENTATION AND CERTIFICATION ARE REQUIRED FOR THIS ITEM.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:     On or about January 9, 2014, employees at Alumi Tank, Inc., were working with chemicals such as polishing compound and metalworking fluid.  The employer did not develop, implement and maintain a written hazard communication program.  Employees were not trained in the hazards associated with the chemicals.      DOCUMENTATION AND CERTIFICATION ARE REQUIRED FOR THIS ITEM.

29 CFR 1904.1(a)(2): If your company had more than ten (10) employees at any time during the last calendar year, you must keep OSHA injury and illness records unless your establishment is classified as a partially exempt industry under 29 CFR 1904.2.   On or about January 9, 2014, Alumi Tank, Inc., did not keep OSHA injury and illness records for the 2013 calendar year.     THIS ITEM WAS CORRECTED DURING THE INSPECTION.
