29 CFR 1926.651(f): A warning system such as barricades, hand or mechanical signals, or stop logs was not utilized when mobile equipment was operated adjacent to an excavation, when such equipment was required to approach the edge of an excavation: 189 Attucks Lane, 189 Attucks Lane, Employee was exposed to crushing hazards when a Caterpillar Model 247B3 Serial Number CAT0247BLTSL01831 tracked skid steer backed up and fell into an excavation where employees were working.
