29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about January 13, 2014, employees performing roofing activities accessed the roof area with ladders not extended at least 3 feet above the upper landing surface and were exposed to the hazard of falling to the ground below.  a.  ladder on the front of the house to the right of the front door to the first level exposed employees to falls of approximately 10 feet to the ground below.  b.  ladder on the front of the house to the left of the front door to the second level exposed employees to falls of approximately 20 feet to the ground below.
