29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest:    (a) 3101 Forest Park Dr Auburn, MA: On or about January 13, 2014, two employees were roofing this residence more than 8 FT above the ground below without any fall protection, which is not in accordance with this subpart.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:    (a) 3101 Forest Park Dr Auburn, MA: On or about January 13, 2014, two employees were accessing the roof of this residence using a ladder that only extended 1 FT past the roof surface, which is not in accordance with this subpart.
