29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)  In the Shop, on or about 1/14/14:  Ingoing nip points at the pressure feed rollers for the Dunning & Boschert four post press #8 were not guarded.    b) In the Shop, on or about 1/14/14: Ingoing nip points at the pressure feed rollers for the Dunning & Boschert four post press #7 were not guarded.    c)  Die Area, on or about 1/14/14:  Rotating shaft and nut projection on a Central Machinery 8 inch bench grinder/buffer were not guarded at the wire brush attachment.    d)  In the Shop, on or about 1/14/14:  Ingoing nip points at the feed roller for Laminator #10 were not adequately guarded.    e)  Pattern Cutting Area #2, on or about 1/14/14:  Rotating chuck on a Dayton drill press was not guarded.    Abatement certification must be submitted for these items.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:    a)  In the Shop at rear door exit #2, on or about 1/14/14:  An orange extension cord providing current to a leaf blower was tie wrapped along conduit and was being used as permanent wiring.    b)  In the Shop near two post press #1, on or about 1/14/14:  Two orange extension cord providing current to portable equipment was attached to the wall surface, strung overhead and being used as permanent wiring.    c)  In the Shop by the mezzanine, on or about 1/14/14:  A strip outlet with extension cords attached being used to power a bandsaw and other extension cords was stapled to the wall and being used as permanent wiring.      Abatement certification must be submitted for these items.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:  a)  In the shop by clicker #9, on or about 1/14/14:  There was dust accumulation on the walls and along the electrical equipment behind clicker press #9.  b)  Pattern Cutting Area #1, on or about 1/14/14:  There were extension cords piled on the floor in front of electrical equipment and a vertical bandsaw, creating a tripping hazard.  Abatement certification must be submitted for these items.
