29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.     (a)    Roof - On or about January 8, 2014 employees were exposed to hazard of being struck by flying objects when using pneumatic nail guns. Employees were not wearing protective eye wear.          (b)   Back of House - On or about January 8, 2014 employees were exposed to hazard of being struck by flying objects when using circular saws.  Employees were not wearing protective eye wear.

(a)  House - On or about January 8, 2014 employees were exposed to a fall hazard while working on a wooden scaffold with walkway less than 18 inches.  The scaffold walkway was only 5.5 inches wide and 7 feet 2 inches above the ground.

29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.     (a)   Roof - On or about January 8, 2014 employees were exposed to fall hazard while working on a steep roof 23 feet 5 inches above the ground with no guardrail system, safety net system or personal fall arrest system.  The roof pitch varied from 9/12 to 12/12.
