29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: (a) Location - 52 Domino Drive, Concord, MA: On or about 1/14/2014, the power cord of a Build All Corporation Part Washer Degreaser had worn in a way that strain relief was not provided to prevent tension from being applied directly to the terminal screws of the cord cap.
