29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a)  NOTE TO CSHO: USE THIS OPTION ONLY IN CASES WHERE THE INFEASIBILITY/GREATER HAZARD EXCEPTION IS NOT IN ISSUE.    On or about January 14, 2014, on the west side of the residential construction site, employees engaged in roofing activities on a 10:12 pitch roof were exposed to the hazard of falling more than 10 feet to the lower level.   MR. JOSE RENEE SIFUENTES WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 316094739, CITATION NUMBER 1, ITEM NUMBER 002 AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 8, 2012, WITH A FINEAL ABATEMENT DATE OF OCTOBER 15, 2012, WITH RESPECT TO A WORKPLACE LOCATED AT 6531 PRAIRIE FLOWER TRAIL, DALLAS, TEXAS 75227.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.    On or about January 14, 2014, on the west side of the construction site, employees using an extension ladder that did not have the side rails extended at least 3 feet over the edge of the roof were exposed to the hazard of falling more than 10 feet to the lover level.   MR. JOSE RENEE SIFUENTES WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.1053(b)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 316094739, CITATION NUMBER 1, ITEM NUMBER 003 AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 8, 2012, WITH A FINAL ABATEMENT DATE OF OCTOBER 12, 2012, WITH RESPECT TO A WORKPLACE LOCATED AT 6531 PRAIRIE FLOWER TRAIL, DALLAS, TEXAS 75227.
