29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous.    (a) Location - 1 Assembly Square Drive, Somerville, MA:  On or about 1/14/2014, an electrical extension power cord that was being used was missing the ground prong.    Lighthouse Masonry, Incorporated was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1926.404(f)(6) which was contained in OSHA inspection Number 31405197, Citation Number 01, Item Number 002, Issued on 11/17/2010 with a final order date of 12/10/2011 with .respect to a workplace located at 56 Federal Street, Salem, Ma.
