29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     (a) Roof, townhouse located at 1718 Cumberland Valley Drive SE, Smyrna, GA 30080: Employees engaged in residential roofing activities on a 3/12 pitched roof at heights of approximately 17-feet, 8-inches were not provided with a fall protection system.

29 CFR 1926.552(a)(1): The employer did not comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators.     (a) Front side, townhouse located at 1718 Cumberland Valley Drive SE, Smyrna, GA 30080: Employer allowed employees to use the TranzSporter (Model LH200) Platform Hoist as a ladder to access the roof. Manufacturer's general safety safety instructions state that this hoist should never be used as a ladder.
