Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushed-by hazards: On or about 1/14/2014, Green Organic Recycling Area, the CATERPILLAR wheel loader, Model 962G, S/N 03BS00816, was operated with a non-functioning horn and back-up alarm, with broken back-up bulbs and lenses, with a broken rear view mirror and a leaking bucket hydraulic cylinder.
