29 CFR 1910.26(c)(3)(iii): Ladders were used on surfaces which did not provide a secure footing: a) On or about January 16, 2014 and at times prior, an employee got injured from slipping and falling off a ladder. The employees used an A-frame ladder to access a 9 foot mezzanine. The ladder was not fully extended and was leaning on the wall which caused the employee to slip and fall from the ladder.
