29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:      On or about January 14, 2014 the employer did not ensure personal protective equipment was provided to protect employees from falling into an approximately 10 foot deep sewage collection pit, when cleaning the walls of the pit.      On or about January 14, 2014 the employer did not ensure personal protective equipment was provided to protect employees from falling from the nursing home facility roof when performing maintenance on the rooftop air handler units.  The nursing home facility rooftop air handler units are located in areas with a maximum eave height of approximately 9 feet, 6 inches, with a roof pitch of approximately 4/12.
