29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.       A. Timothy J. Wilson Construction, LLC worksite located on Allentown Rd., Lima, Ohio: On or about January 14, 2014, the employer did not ensure employees were wearing proper eye protection while operating pneumatic nail guns while performing framing activities on the single story wood framed commercial structure. The employees were exposed to potential flying object hazards while operating the pneumatic fasteners.

Wilson Construction, LLC worksite located on Allentown Rd., Lima, OH: On or about January 14, 2014, the employer failed to ensure the top of an 8 ft A-framed stepladders was not used as a working level by the employees. The employees were using the top step of the ladder to perform framing activities exposing the employees to potential fall hazards.

29 CFR 1926.501(b)(1) Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.     A. Timothy J. Wilson Construction, LLC worksite located on Allentown Rd., Lima, Ohio: On or about January 14, 2014, the employer did not ensure employees were protected from fall hazards while performing framing activities on the roof of a single story wood framed commercial structure. The exposed employees were installing a parapet on the north end of the structure exposed ot a fall distance of approximately 14.00 feet.     Timothy J. Wilson Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501 which was contained in OSHA inspection number 311610539, citation number 1, item number 3 and was affirmed as a final order on September 27, 2009, with respect to a workplace located at Elida, Ohio.
