29 CFR 1910.146(c)(5)(ii)(C): Before an employee enters the space, the internal atmosphere shall be tested, with a calibrated direct-reading instrument, for oxygen content, for flammable gases and vapors, and for potential toxic air contaminants, in that order. Any employee who enters the space, or that employee's authorized representative, shall be provided an opportunity to observe the pre-entry testing required by this paragraph.  a.     On the site where employees entered confined spaces (Grain Bins) to clean the inside prior to refilling the bins with new product, the employer did not use a atmosphric tester to check the oxygen content prior to entering the bin thereby exposing employees to an inhalation hazard.

Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.  a.     In the mill work room, the front of the roller mill had a unguarded belt and pulley thereby exposing employees to a caught-in hazard.  b.     In the mill room, the rear of the roller mill had two unguarded  belts and pulleys thereby exposing employees to a caught-in hazard.  c.      In the basement chemical room, there was an unguarded belt and pulley on the grain prober, thereby exposing employees to a caught-in hazard.
