29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) Meadowview Townhomes - On or about 17 January, 2014 - Employees were working on a working surface 6 feet or more above the lower level without the use of a fall protection system in place.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  a) Meadowview Townhomes - On or about 17 January, 2014 - Employees were using an extension cord to power hand tools that did not have strain relief to prevent pull from being directly transmitted to joints or terminal screws.
