29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.    a)Louisville Hotel & Suites, LLC dba Best Western @ 3737 Quebec Street, Denver CO 80207: On and before January 17th, 2014, the employer did not ensure that the laundry room was kept clean and in a sanitary condition. Employees were exposed to unsanitary conditions in the form of broken floor tiles, lint, trash, dirty laundry, and old equipment on the floor throughout the laundry room; particularly the areas behind the washers and the dryers.  Heavy lint build-up was allowed to accumulate on top of dryers, inside the lint bin located under the dryer, and all over the floor.  The condition in the laundry room exposed employees to slips, trips, falls and a fire hazard because of the excessive lint accumulation.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):       (a)Louisville Hotel & Suites, LLC dba Best Western @ 3737 Quebec Street, Denver, CO 80207: On and before January 21st, 2014, the employer did not assess the workplace to determine if hazards were present, or likely to be present, which necessitate the use of PPE. The employer has not performed a PPE hazard assessment.  Employees were working with StainBlaster A, Stainblaster B, and Tri-Star Solar Brite without the use of gloves. This condition exposes employees to hazards to extreme skin irritation.       Abatement note: The employer shall verify that the required workplace hazard assessment has been performed through a written certification that shall include the following:       (1)          Identifies the workplace evaluated;   (2)          The person certifying that the evaluation has been performed;   (3)          The date(s) that the hazard assessment was performed; and   (4)          Which identifies the document as a certification of hazard assessment.
