29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system:  An employee for Wilder Construction was exposed to a fall hazard approximately 11 feet 6 inches while installing siding material and fall protection was not provided or used.

29 CFR 1904.39(a): An oral report of an employment accident resulting in a fatality or the inpatient hospitalization of three or more employees was not made within eight (8) hours after the occurrence to the nearest Area Office of the Occupational Safety and Health Administration or to the OSHA toll-free central telephone number (1-800-321-OSHA):   On or about December 31, 2013 an employee of the employer was pronounced dead because of severe head injuries due to a fall from height at 1202 Silver Oak Circle, Normal IL, and the employer did not make an oral report to the Peoria Area OSHA Office, within 8 hours of the death of an employee.
