29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed:  On or about September 25, 2013, and at other times prior thereto , at the production facility in Karnes County, TX, the employer failed to have written certification that a hazard assessment had been conducted to determine what type of personal protective equipment was required to protect the employees when taking air monitoring samples in front of a problematic H2S, Ultrafab scrubber.
