29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   (a) On or about 1/22/2014, at the front of the building, an employee standing on the rails of the scissor lift basket was exposed to falls of approximately 20 feet to the concrete below.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about 1/22/2014, on the front of the building, employees working at the edge of the building without fall protection were exposed to falls of approximately 20 feet to the concrete below.
