29 CFR 1910.132(e): The employer did not ensure that employees were not using defective or damaged personal protective equipment in the workplace: Fire resistance (FR) clothing used by employees tasked with completing service or maintenance activities on energized equipment, was not properly maintained. The FR clothing had holes throughout the garment(s), and the clothing was not properly laundered to ensure the material maintained its protective qualities.
