29 CFR 1910.132(e): The employer did not ensure that employees were not using defective or damaged personal protective equipment in the workplace:     Fire resistance (FR) clothing used by employees tasked with completing service or maintenance activities on energized equipment, was not properly maintained.  The FR clothing had holes throughout the garment(s), and the clothing was not properly laundered to ensure the material maintained its protective qualities.

29 CFR 1910.335(b)(1):  Safety signs, safety symbols, or accident prevention tags were not used where necessary to warn employees about electrical hazards which may endanger them:       On or before 01/09/2014, arc flash labels were not affixed to the feeder and branch circuits. The unlabeled equipment includes, but in not limited to, the wing bases that branch from the primary 480 volt electrical distribution panel box of the Block line, in department 4662.
