29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   On or about 22 January 2014, and at times prior thereto, two framers were using pneumatic nail guns to perform framing activities without the use of eye protection, exposing the workers to the hazard of being struck by flying objects or projectiles.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step.   On or about 22 January 2014, and at times prior thereto, two framers were each using a Werner 6 foot fiberglass step ladder to perform framing activities from while standing on the top step or the second step down, exposing each framer to a fall hazard to the concrete foundation below.
