29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person.   On or about 22 January 2014, and at times prior thereto, a scaffold walkway and walking/working platform were not constructed by a qualified person in that the width of the platform was approximately 11 inches wide at a height of approximately 4 feet 8.5 inches, exposing the framer using the platform to a fall hazard to the concrete foundation below.

29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and walkway was not at least 18 inches (46 cm) wide.  On or about 22 January 2014, and at times prior thereto, a framer was using a scaffold walkway and platform to perform framing activities that was not at least 18 inches wide, exposing the framer to a fall hazard to the concrete foundation below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).   On or about 22 January 2014, and at times prior thereto, a framer was performing framing activities on a residence under construction at the top plate without fall protection, exposing the framer to a fall hazard of approximately 10 to 12 feet to the concrete foundation below.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder cannot be used as a step.   On or about 22 January 2014, and at times prior thereto, a framer was using an 8 foot fiberglass step ladder to ascend or descend from the top plate by using the top step, exposing the framer to a fall hazard of approximately 8 or more feet to the concrete foundation below.
