29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. In the metal prefabrication shop employees were exposed to an amputation hazard, in that, no guards were present while employees used the Fabriline hydraulic punch . On or about 01/27/2014. b. In the metal shop employees were exposed to an amputation hazard, in that, the blade on the Wellsaw horizontal band saw model 1016 sn #2695 was not guarded while employees cut angle iron.
