29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  a. At the facility located on 675 Cypress Drive, Merritt Island, FL, employee exposed to the health hazards associated with a chrome electroplating operation was wearing a dual stap, dust/mist respirator. The respirator was not adeqaute for the employee exposure to acid vapors, on or about, 01/29/2014.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user  a) for the employee (owner) at the facility located on 675 Cypress Drive, Merritt Island (FL), exposed to the health hazards associated with a chrome electroplating operation, as observed on or about, 01/29/2014.  The employee was wearing a dual stap, dust/mist respirator but a written respirator program had not been developed and implemented to address elements such as but not limited to: proper selection, medical evaluations, training, and proper storage.
