29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was required to use the respirator in the workplace: (a) Diamond Blast Corporation did not provide a medical evaluation to detemine the employee(s) ability to use a respirator before the employee(s) were required to use the respirator. Employees utilizing an air-line respirator were not medically evaluated prior to using the respirator in the workplace. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): (a) On or about March 11, 2014, Diamond Blast Corporation did not ensure that unused circuit breaker(s) were properly guarded. An unused circuit breaker was improperly guarded with duct tape. No abatement certification or documentation is required for this item.
