The fabricated extension cord (with an outlet box as the receptacle) was used with a cover on the plug end, that was not in place (loose, potentially exposing terminal connections to the plug blades).  (Note: there is no longer any approval for plugs with this type of cover, but must be of a solid construction.)  Abatement documentation is required for this violation.
