29 CFR 1910.303(b)(1)(viii): Electric equipment was not free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment was not determined using the following considerations: Other factors that contribute to the practical safeguarding of persons using or likely to come in contact with the equipment: (a) At Springfield Elfindale, LLC on or about January 24, 2014 the 110 volt duplex outlet used to provide power to a fountain located near the nurses station was not protected by a ground-falut circuit interrupter. (b) At Springfield Elfindale, LLC on or about January 24, 2014 the 110 volt duplex outlet used to provide power near the water outlet in the Central Supply was not protected by a ground-fault circuit interrupter. and (c) At Springfield Elfindale, LLC on or about January 24, 2014 the 110 volt duplex outlet in the walk in freezer was not protected by a ground-fault circuit interrupter.
