29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.    a/ Preferred Freezer Services FFD, LLC/Perth Amboy NJ Site - Warehouse/Freezer: A powered industrial truck operator was  not provided formal instruction as required by the standard.     Condition observed on/or about 1/27/14.

29 CFR 1904.32(a)(2): 29 CFR 1904.32(a)(2):  At the end of each calendar year an annual summary of injuries and illnesses recorded on the OSHA 300 log was not completed in detail.    a/ Preferred Freezer Services FFD, LLC/Perth Amboy NJ Site: Annual summary was not completed according to the requirements of the standard.    1- Annual summary year 2013: Column H and I indicated 2 injuries were recorded which is different from total number of injuries and the OSHA 300 log for the same year.    2-Annual summary year 2012: Column H, I, J and total number of injuries indicated 3 injuries were recorded which is different form the OSHA 300 log for the same year. Total number of days away from work and total number of days of job transfer or restriction is different from the OSHA 300 log for the same year.    3-Annual summary year 2011: Annual average number of employees and total hours worked by all employees is missing.
