29 CFR 1910.178(l)(3)(i)(M): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in paragraph (1):    190 NJ Theriot Road, Golden Meadow, Louisiana; on or about January 24, 2014, the employer failed to ensure that employee(s) had received all of the training required by paragraph (1) of this section prior to operating powered industrial trucks.  Employee(s) were not trained on the following:    1.  Hazards associated with turning a forklift such as the tail swing.  2.  keeping a clear view of the path of travel when operating a forklift.

29 CFR 1910.178(n)(6): Industrial truck drivers shall be required to look in the direction of, and keep a clear view of the path of travel:   On or about January 24, 2014, at 190 NJ Theriot Road, Golden Meadow, LA 70357, an employee was operating a forklift to load cargo onto an 18 wheeler truck.  Upon the completion of loading the truck, the operator proceeded to park the forklift without a clear view of the path of travel and struck a truck driver.
