29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: a) Keen Pump Company, Inc. - Ashland, Ohio: On or about February 3, 2014, the employer did not assure that the flexible power cord that was wrapped around the Dayton #24-1 chain hoist (located on the I-beam labeled "K") was free from electrical hazards by assuring equipment was in good repair. Employees were exposed to electrical hazards in that the cord's protective outer cover was being pulled from the plug end of the cord.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Keen Pump Company, Inc. - Ashland, Ohio: On or about February 3, 2014, the employer did not assure that employees were protected from electrical hazards when operating the electrical test panel located in the assembly area to test pumps. The employer installed an extension with alligator clips to test blocks, exposing employees to live parts up to 480 volts. The test panel was not used in accordance with listing instructions in that pumps were not directly connected and secured to test blocks during the electrical test.
