29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: On or about February 3, 2014, the employer Fox River Food Service dba Performance Food Service, did not perform an evaluation of each powered industrial truck operator's performance as required once every three years. Two employees were not re-evaluated. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
