29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner: The employer does not ensure that scaffold casters and wheels are locked to prevent movement of the scaffold while the scaffold is in use. This violation occurred on or about January 30, 2014 at a workplace located at 3205 Rocking Hills Trail, Forney, TX; where an employee was exposed to a fall hazard when working from a mobile scaffold that did not have the casters locked.
