29 CFR 1910.1048(d)(2)(ii): The initial monitoring process was not repeated each time there was a change in production, equipment, process, personnel, or control measures which could have resulted in new or additional exposure to formaldehyde:      (a) Shear Bliss Salon introduced a new hair treatment, Acai Professional Smoothing Solution, into  the worksite; the employer did not perform initial monitoring to determine possible exposure to formaldehyde from the newly introduced hair treatment. The condition was noted on or about January 29, 2014.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER  IS REQUIRED TO SUBMIT ABATEMENT  DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 AS PER 29 CFR 1903.19.

29 CFR 1910.1048(n)(2): The employer did not provide employees with information and training on formaldehyde at the time of their initial assignment and/or wherever a new hazard from formaldehyde was introduced into their work area:    (a) Shear Bliss Salon introduced a new hair treatment, Acai Professional Smoothing Solution, into  the worksite; the employer did not provide additional training when the new hair treatment was introduced. The condition was noted on or about January 29, 2014.   NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER  IS REQUIRED TO SUBMIT ABATEMENT  DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 AS PER 29 CFR 1903.19.
