29 CFR 1910.252(a)(2)(vi)(C): Cutting or welding was permitted in the presence of explosive atmospheres (mixture of flammable gases, vapors, liquids, or dusts with air), or explosive atmospheres that could develop inside uncleaned or improperly prepared tanks or equipment which had previously contained such materials, or that could develop in areas with an accumulation of combustible dust:  a) for the facility located at 8813 US Hwy 301 S., Riverview, FL.  Employees were performing welding activities at sulfuric acid plant #7 where explosive gases were being generated upstream.  The accumulating gases created an explosive atmosphere which were ignited by the welding task and resulted in an explosion, on or about 11/23/2013.

Upstream plant operations were initiated prior to welding being completed at sulfuric acid plant #7 exposing employees to an explosive atmosphere.  An explosion did occur on or about, 11/23/2013.
