29 CFR 1915.73(e): When employees were working near the unguarded edges of decks of vessels afloat, they were not protected by personal flotation devices meeting the requirements of 29 CFR 1915.158(a).  On or about 21 Jan 2014, the employer did not ensure that employees would not be exposed to fall hazards while working aft on the SHI LADY II, and were not provided with personal flotation devices.
