(a) This most recently occurred on February 4, 2014, at 101 North Main Street, Newark, Illinois. Employees are exposed to caught in-between amputation hazards on the SteelWeld 500 Ton mechanical press brake model no. M06-12, when hand-feeding and aligning metal parts in the unguarded point of operation for regular bending fabrication tasks.   (b) This most recently occurred on February 4, 2014, at 101 North Main Street, Newark, Illinois. Employees are exposed to caught in-between amputation hazards on the Cincinnati mechanical shear #20 model no. 2510, serial no. 19745, when hand-feeding and aligning metal parts in the unguarded point of operation for regular shearing fabrication tasks where the blade and hold downs were not guarded.  This employer has been cited for this practice that included: Dierzen Trailer. Co., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii) which was contained in OSHA violation number 314040320, citation 2, item 1, issued on February 9, 2011, and became final order on or about March 7, 2011, with respect to a workplace located at 101 N. Main St., Newark, IL 60541 , and OSHA violation number 309768497 citation 1, item 2, issued on January 26, 2007, and became final order on or about February 13, 2007, with respect to a workplace located at 101 N. Main St., Newark, IL 60541.  Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including an explanation of how these steps protect its employees from caught in-between amputations.
