29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.      On or about January 30, 2014, on the west side of the construction site, employees were installing roofing material with the use of pneumatic nail guns and were not wearing eye protection exposing employees to eye injuries.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.       On or about January 30, 2014, on the west side of the construction site, employees engaged in roofing activities on a 6:12 pitch roof worked without fall protection and were exposed to a fall hazard of approximately 25 feet.
